Key Responsibilities
- Maintain a caseload of clients enrolled in the nutrition program.
- Provide counseling to clients regarding program options, employing various techniques such as active listening and goal setting.
- Determine client eligibility and customize food packages; issue benefits for grocery store redemption.
- Evaluate anthropometric data and assess nutritional status through comprehensive interviews.
- Respond to inquiries from clients and the public, offering information about county programs and services.
- Utilize specialized data entry systems for benefit issuance and maintain accurate records.
- Schedule appointments and track client adherence to ensure effective follow-up.
- Refer clients to additional support services, including social services and food assistance programs.
- Engage in outreach initiatives and participate in community events.
- Deliver exceptional customer service to clients from diverse backgrounds.
- Make referrals to Registered Dietitians as needed.
- Participate in departmental emergency planning and response efforts.
- Promote public health initiatives within the community.
Knowledge, Skills, and Abilities:
- Effective communication and organizational skills.
- Ability to work collaboratively and maintain professional relationships.
- Strong multitasking abilities and time management skills.
- Capacity to adapt to changing situations and reprioritize tasks.
- Proficient in maintaining confidentiality and managing sensitive information.
- Skilled in using computer software, including Microsoft Office.
- High school diploma or equivalent.
- At least two years of experience in community health, public health, or related fields.
- Valid driver's license required.
- Ability to obtain necessary certifications within specified timelines.
The work environment may involve exposure to various health-related conditions. Some travel may be necessary, and duties may be performed in multiple locations. Candidates must be able to lift and carry up to 35 pounds and engage with clients at their level.
