Responsibilities- Assess and Develop Treatment Plans: Conduct thorough patient evaluations and develop individualized treatment plans that align with physician orders and address the patient's overall medical condition.
- Implement Patient Care Plans: Carry out patient care plans or portions thereof, determining the most effective progression of patients within the established care plan.
- Perform Tests and Treatments: Conduct tests and complex treatments individually or with assistance as needed, ensuring the highest level of patient care.
- Collaborate with Healthcare Team: Work closely with occupational therapists, physical therapists, physicians, and other healthcare professionals to develop and implement comprehensive patient treatment programs.
- Document Patient Progress: Maintain accurate and timely documentation of patient evaluations, treatment progress notes, and discharge summaries in accordance with department policy.
- Provide Speech Therapy Services: Deliver a full range of speech therapy modalities consistent with previous education and experience, ensuring the best possible outcomes for patients.
- Master's Degree in Communication Disorders: Possess a Master's degree in communication disorders or a related field.
- State License: Hold a current and valid state license in good standing.
- Experience in Skilled Nursing Facilities: Preferably one year of experience in skilled nursing facilities and/or long-term care settings.
