Key Responsibilities- Program Development and Implementation: Collaborate with Chief Physicians, staff physicians, and managers/care providers from multiple disciplines/departments to develop and implement disease management programs that meet access, cost, quality, and member/patient satisfaction goals and needs.
- Care Management: Provide comprehensive on-going care management that meets established standards/benchmarks for efficiency, improved clinical outcomes, quality, resource utilization, cost effectiveness, evidence-based treatment plans, and improves members' quality of life and satisfaction.
- Program Evaluation and Improvement: Analyze program outcomes actually achieved vs intended outcomes, monitor and resolve program compliance issues involving providers and patients, and evaluate disease management program in terms of utilization review/resource issues.
- Collaboration and Communication: Act as organization liaison with relevant community agencies, organizations, and task forces, and collaborate with Chief Physicians and care providers/managers to develop and implement clinical pathways and care management interventions.
- Experience: Minimum four (4) years of behavioral health case management experience in a relevant chronic disease in an acute medical center, or minimum four (4) years of experience analyzing/improving behavioral health care clinical practices/processes and clinical performance in an acute medical center.
- Education: Masters degree in clinical social work from an institution accredited by the Council on Social Work Education.
- License, Certification, Registration: Licensed Clinical Social Worker (California).
