This role is essential in ensuring that high-quality care is provided through the supervision of staff performance, communication with physicians, care planning, and daily resident assessments. The Licensed Practical/Vocational Nurse will also deliver direct care to residents, adhering to nursing standards, established procedures, and regulatory requirements for skilled nursing facilities.

Key Responsibilities:
  • Oversee daily operations and monitor the performance of nursing assistants, ensuring exceptional nursing care and task completion.
  • Facilitate the orientation of new LPN/LVNs and nursing assistants, reinforcing training standards to uphold care quality.
  • Provide clear explanations of nursing procedures and assist staff to ensure ongoing training and understanding.
  • Maintain regular communication with the Nurse Manager or Director of Nursing regarding observations, suggestions, and resident activities.
  • Assess and evaluate residents' health, mental, and emotional conditions, promptly notifying appropriate staff of significant changes.
  • Deliver direct patient care, including procedures such as catheter placement, colostomy care, suctioning, dressing changes, and naso-gastric tube management.
  • Participate in shift reports to ensure accurate information exchange about residents.
  • Assist with the admission and orientation of new residents, ensuring physician orders for diets, therapy, and medications are accurately transcribed and communicated.
Qualifications:
  • Graduate from an accredited school of licensed practical/vocational nursing.
  • Current, valid LPN/LVN license in good standing.
  • Minimum of one year of nursing experience.
  • Commitment to the principles and standards of professional nursing.
