Job Overview

We are seeking a highly skilled Compassionate Primary Care Specialist to join our team at Quartet Health.

As a Compassionate Primary Care Specialist, you will play a vital role in providing high-quality, patient-centered care to adults facing complex medical and behavioral health conditions.

Key Responsibilities:

  • Develop individualized care plans and assess patient progress, working collaboratively with the interdisciplinary care team.
  • Provide expertise in general medicine and evidence-based behavioral health care, consulting with the broader internal team as needed.
  • Perform initial intake visits, using assessment instrument criteria while engaging other care team members as required.
  • Order, interpret, and analyze diagnostic tests, recommending follow-up appointments and further treatment options as necessary.
  • Prescribe and manage medications and treatments, including injectables, as appropriate.
  • Capture diagnosis details, including billing codes, and submit claims.
  • Identify social needs and non-medical barriers to care for patients and collaborate with the Care Team to address.
  • Manage the psychosocial component of each patient's care.
  • Engage actively with members of the interdisciplinary team via frequent patient rounding to drive efficient care planning.
  • Initiate internal referrals to engage other expertise within the Quartet Care Team.
  • Develop strong rapport with diverse patients, both in-person and via telehealth, supporting patient engagement in the program.
  • Uphold clinical customer service through the delivery of high-quality, empathetic care.
Requirements

To be successful as a Compassionate Primary Care Specialist, you will need:

  • An active nurse practitioner license and certification in good standing with the New Jersey Board of Nursing.
  • Expertise in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions.
  • The ability to travel locally at least 75% of the time to see patients in their homes and communities.
  • A valid driver's license, active auto insurance, access to a car, and willingness to drive to patient locations.
  • Excellent written and verbal communication skills, with an emphasis on clarity and compassion.
  • Strong computer skills, including experience working with Electronic Health Records (EHR).
  • A commitment to consistently modeling Quartet's values, including teamwork, patient-centricity, innovation, excellence, and productivity.
Preferred Qualifications

We welcome candidates with:

  • Experience in home-based services, complex care cases, or caring for patients with co-occurring substance use disorders.
  • Backgrounds in value-based care models.
  • Previous telehealth experience.
  • Skills in using online tools and technology to deliver care and communicate with patients.
  • Dual FNP/PMHNP backgrounds.
  • Collaborative work styles, with a focus on effective teamwork.
  • Bilingual abilities (Spanish and/or other languages).
Compensation and Benefits

We offer a competitive salary range of $120,000 - $160,000 per year, depending on experience, as well as a comprehensive benefits package that includes medical, dental, and vision coverage, a generous paid time off policy, and opportunities for professional growth and development.
