Job Title: Continuing Care and Alumni Coordinator

Job Summary:

The Continuing Care and Alumni Coordinator will play a vital role in ensuring the successful transition of guests from our residential recovery program to long-term recovery. This position involves creating a continuum of care, overseeing alumni engagement, and developing programs to keep former clients connected to our recovery community.

Key Responsibilities:

  • Develop personalized continuing care plans in collaboration with our team, guest family, and referring professionals.
  • Maintain contact with referral sources to update them on guest progress and collaborate on continuing care plans.
  • Communicate with chosen providers to set post-discharge appointments and ensure guests understand their plans and arrangements.
  • Facilitate client satisfaction survey data collection and use results to improve aftercare services.
  • Develop and maintain relationships with external providers and educate clients on relapse prevention strategies and community resources.

Alumni Coordinator Responsibilities:

  • Develop and maintain an alumni program, providing ongoing support and resources to former guests.
  • Introduce guests to the alumni program upon admission and obtain consent for communication post-discharge.
  • Organize in-person and virtual meetings, activities, and events for alumni, their families, and friends to strengthen the alumni community.
  • Perform well-being check calls and texts to discharged alumni on a prescribed schedule.
  • Assist with the development and production of signature meetings and events, as well as develop new and enhance existing alumni and family traditions.

Requirements:

  • Bachelor's degree in human services or a relevant field is preferred.
  • 2+ years of experience in substance use disorder treatment, case management, or alumni relations.
  • Ability to work collaboratively on a multidisciplinary team and build rapport with clients, alumni, and families.
  • Knowledge of continuing care resources and strong organizational and case management skills.
