We are looking for a highly skilled Clinical Support Specialist to join our team at Bread for the City. As a Community Health Nurse, you will play a critical role in providing clinical support to our front desk staff and health resource room. Your responsibilities will include providing health teaching, assisting with follow-up of patients after Emergency Department visits and hospital discharges, and developing, coordinating, and participating in ongoing training for clinic staff on risk management and safety.

This position requires an individual with strong organizational skills, who can work collaboratively with our healthcare providers and other staff members to ensure seamless delivery of services. You must be able to communicate effectively with patients from diverse backgrounds and have experience working in a fast-paced environment.

The estimated salary for this position is $60,000-$80,000 per year, depending on experience.
