Job Title: Resource Specialist

Pyramid Consulting, Inc. is seeking a highly skilled Resource Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing care management services to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits, and providing community resources to address social determinants of health needs.

Key Responsibilities:
  • Provide field-based care management services to a caseload of 100 members, including home visits, group home/hospital settings, and community/agency meetings.
  • Assess health care needs, refer to health plan providers, and provide education on benefits and expanded benefits.
  • Document in an electronic health record, including outreach notes, assessments, and care plans.
  • Comply with care management policies and procedures, including outreach and documentation expectations.
  • Participate in monthly audits to ensure charts comply with regulations.
Requirements and Technology Experience:
  • Pediatric case management experience.
  • Experience connecting families/children to local resources for various services, including employment, housing, independent living, justice, foster care, and adoption.
  • Health plan experience.
  • Foster care and/or adoption experience.
  • Bachelor's Degree required.
  • Microsoft Office skills, including Outlook, Word, and Excel.
