We are seeking an experienced Medical Assistant to join our team as a Patient Care Coordinator. As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and support to our patients and their families.
Key Responsibilities- Provide education and information to patients and their families regarding treatments, procedures, and continuing care.
- Explain medication(s) and treatment plans to patients and their families.
- Collaborate with various departments to ensure accurate scheduling, lab results, and recordkeeping.
- Answer and address incoming patient clinical calls, RX, and PA.
- Type up phone notes/cases and route to providers regarding patient concerns.
- Place phone calls to patients regarding answers to concerns.
- Call and inform patients of pathology/lab results.
- Schedule patients for surgeries (excision, etc.).
- Retrieve patient information from PCP or specialist's office.
- Obtain patient results from affiliated labs.
- Fax reports per request of outside providers.
- Maintain the PA/RX buckets.
- Support and assist with ePath and procedure scheduling as needed.
- Check patient portal messages and route to provider accordingly.
- Ensure accurate notes of all consultations and treatments are recorded in the patient's record.
- Communicate effectively and tactfully with staff, consultants, patients, significant others, and community.
- Reduce conflict and increase patient satisfaction through timely response to calls.
- Prioritize work tasks and services and complete tasks in a timely manner.
- Complete Administrative Proficiency (MS Word, Excel, PowerPoint, Web-based applications, email, HRIS systems).
- Certification as a Medical Assistant is required.
- Triage experience is preferred.
- Dermatology experience is preferred.
