The Virginia Garcia Memorial Health Center is dedicated to delivering high-quality, culturally sensitive healthcare to underserved populations in Washington and Yamhill Counties, with a focus on seasonal and migrant farm workers, as well as individuals facing barriers to accessing medical services.

Role Summary: The Physician Associate/Nurse Practitioner will deliver primary care medical services to patients in alignment with their training, experience, and licensure. This role involves collaboration as part of the medical team within the Center and serves as an educational resource for patients, the Center, students, and the broader community.

Key Responsibilities:

  • Engage in direct patient care, which may encompass obstetrics, family planning, newborn care, pediatrics, and general adult medicine.
  • Achieve minimum patient care productivity standards set by the Center to fulfill external funding and reimbursement requirements.
  • Deliver continuous, comprehensive, and culturally competent healthcare of the highest standard.
  • Manage cases for Center patients effectively.
  • Document all necessary information in the medical records, including education, medications, referrals, diagnostic tests, and follow-ups.
  • Assist in organizing and participating in specialized clinics, such as those for migrant camps.
  • May be required to provide services at various locations beyond the primary worksite.
  • Supervise support staff regarding individual patient management on a daily basis.
  • Participate in case management discussions and reviews, including those for obstetric patients and individuals with diabetes.
  • Contribute to the implementation of plans aimed at achieving the objectives of the annual Health Plan and other goals set by the QA/QI committee and the Center's managed care initiatives.
  • Engage in chart audits to evaluate the effectiveness of the QA/QI program.
  • Participate in medical staff meetings, OB meetings, QA/QI meetings, and other committees focused on enhancing patient care and the overall operations of the Center.
  • Provide mentorship to students and residents in healthcare professions and engage with community groups to foster the Center's integration into the community.
  • Support the Medical Director or CEO in fulfilling the Center's mission.
  • Perform additional duties as assigned.
  • Handle protected health information (PHI) in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

HIPAA Compliance:

The licensed independent healthcare practitioners will access PHI while performing their duties, utilizing it for treatment and quality assurance operations. In adherence to the minimum necessary standard of HIPAA, the designated records accessible to this employee include demographic and scheduling information within the practice management system, as well as the complete medical record.

Required Knowledge, Skills, and Abilities:

  • Bilingual proficiency in Spanish and English is preferred; willingness to attain sufficient Spanish language skills to provide medical care is essential.
  • A strong commitment to serving patients and collaborating with staff from diverse ethnic, economic, cultural, and social backgrounds.

Education and Experience Requirements:

  • Completion of an accredited family nurse practitioner or physician associate training program recognized by the State of Oregon.
  • Licensure by the State of Oregon through the Board of Nursing or the Board of Medical Examiners.
