Role Summary:
The Social Worker will deliver a comprehensive range of clinical social work services within a primary care or home health environment. This includes offering support and guidance to the care team in managing patient needs. The professional will assess psychosocial strengths and challenges to facilitate patient independence and collaborate with the team to develop effective care plans. Responsibilities also include providing short-term counseling, education, and connecting patients with necessary resources.
Key Responsibilities:
- Implement appropriate social work interventions for patients of all ages and their families, which may include psychosocial assessments, individual or family counseling, advocacy, referrals, group work, case management, and long-term care planning.
- Engage in consultation and coordination with a multidisciplinary team of healthcare professionals, participating in family and care conferences, team meetings, and other collaborative efforts.
- Assess patient and family requirements for care, ensuring effective communication with other healthcare providers. Collect relevant data during visits and conduct ongoing assessments throughout the care process.
- Evaluate the needs of patients and families through timely assessments and observations, adjusting care plans based on patient responses.
- Minimum of one (1) year of prior social work experience.
- Current KP Coalition employees may have this experience requirement waived.
- Master's degree in social work (MSW).
- Must hold one of the following licenses in Washington: Licensed Marriage and Family Therapist, Licensed Advanced Social Worker, Licensed Social Worker Associate-Independent Clinical, Licensed Independent Clinical Social Worker, or Mental Health Counselor.
- Strong written and verbal communication abilities.
- Proven success in a multidisciplinary team environment and a commitment to diversity.
- Two (2) years of post-master's experience.
- Experience in care coordination within managed care systems, inpatient/outpatient settings, skilled nursing, or long-term care facilities.
- Knowledge of social case management, conflict resolution, and team-building practices.
- Familiarity with Microsoft Office and electronic charting systems.
