Key Responsibilities

  • Identify and facilitate access to community resources, including local, state, and federally funded programs.
  • Provide supportive counseling and education to members, families, and caregivers.
  • Collaborate with the healthcare team to ensure consistent and reliable documentation of case management activities.
Requirements
  • Master's degree in social work (MSW) required.
  • Minimum one (1) year of clinical social work experience in a healthcare setting required.
  • Licensed Independent Social Worker (District of Columbia) or Licensed Graduate Social Worker (District of Columbia) within 6 months of hire.
  • Licensed Master Social Worker (Maryland) or Licensed Master's Social Worker (Virginia) within 6 months of hire.
