Job Summary:
We are looking for a dedicated and compassionate RN to provide exceptional patient care and support to our patients and families. As a Case Manager RN, you will work collaboratively with our healthcare team to enhance the delivery of patient care services along the continuum of care.
Responsibilities:
- Evaluate, predict, and facilitate the trajectory of patient care
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet the individual's health care needs
- Collaborate with healthcare team members to ensure seamless care transitions and optimal patient outcomes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Provide education and support to patients and families to promote health literacy and self-management
Requirements:
- RN Case Manager experience required
- Be a licensed RN in the state of Massachusetts
- Minimum of 2 years Med/Surg experience required
- Case Manager certificate preferred
- ADN required, but BSN strongly preferred
