Job Title: Home Care Physical Therapist

At Liberty Home Care, we are seeking an experienced Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping our patients recover from illness, trauma, or surgery in the comfort of their own homes.

Key Responsibilities:

  • Evaluate patients and develop personalized plans of care to address their specific needs.
  • Provide direct treatment and follow-up care to patients in their homes.
  • Document patient care data accurately and thoroughly, reflecting progress and improvement in patient condition.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care.
  • Collaborate with Patient Care Coordinators to make referrals and coordinate care.
  • Provide education and training to patients, families, and healthcare professionals as needed.

Requirements:

  • Master's degree in Physical Therapy from an accredited program and current licensure in the state where therapy will be provided.
  • Minimum one year of experience in a non-home health care environment.
  • Strong knowledge of physical therapy theory and concepts, with ability to evaluate and treat patients with physician orders.
  • Continuing education requirements met annually.
  • Ability to travel and work in a variety of settings as assigned.
