At Liberty Home Care, we understand the importance of recovery at home after an illness, trauma, or surgery. Our dedicated healthcare professionals strive to provide recovery with independence to our patients.

We are seeking an experienced Home Care Physical Therapist to join our team.

Key Responsibilities:
  • Provide patient care services, including direct treatment and follow-up.
  • Evaluate patients new to service, develop a plan of care, and document patient care data accurately and thoroughly.
  • Communicate effectively with intercompany and intracompany contacts, discuss company and third-party reimbursement policies, and coordinate referrals to appropriate disciplines.
  • Provide in-service education, healthcare professional education, and public education, as well as supervise peers and support personnel.
  • Conduct research and assist with orientation of new employees as needed.
Requirements:
  • Minimum bachelor's degree in physical therapy from an accredited school of physical therapy and current licensure from the state where therapy will be provided.
  • Prefer a minimum of one year's therapeutic experience in a non-home healthcare environment.
  • Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to evaluate and treat patients for whom a physician's order exists.
  • Minimum of twelve hours of continuing education annually.
  • Ability to accept travel and working conditions as assigned.
