Job Summary:

  • Conduct thorough assessments of patients' physical, cognitive, and sensory abilities to identify areas for improvement.
  • Develop and implement treatment plans that are tailored to each patient's unique needs and goals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient care data accurately and thoroughly, reflecting evidence of improvement or decline in patients' conditions.
  • Communicate effectively with patients, families, and healthcare teams to ensure that patients receive the best possible care.

Requirements:

  • Master's or Doctoral degree in Occupational Therapy from an accredited program.
  • Current licensure as an Occupational Therapist in the state where therapy will be provided.
  • Minimum of one year's experience in occupational therapy, preferably in a home care setting.
  • Strong knowledge of occupational therapy principles and practices, as well as excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care.
