Job Title: Home Care Physical Therapist

Job Summary:

Liberty Health is seeking an experienced Home Care Physical Therapist to join our team. As a Home Care Physical Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services in the comfort of patients' homes.

Key Responsibilities:

  • Provide Patient Care Services: Deliver direct treatment and follow-up care to patients in their homes, ensuring a safe and effective recovery.
  • Evaluate and Develop Patient Plans: Assess patients' needs and develop personalized plans of care, taking into account their diagnosis and treatment goals.
  • Document Patient Progress: Accurately and thoroughly document patient care data, reflecting improvements, progress, or declines in patients' conditions.
  • Communicate with Intercompany Contacts: Collaborate with internal stakeholders to ensure seamless communication and coordination of patient care.
  • Discuss Reimbursement Policies: Educate patients and families on company and third-party reimbursement policies, as well as treatment plans with the Patient Care Coordinator, physician, and patient/family.
  • Coordinate Referrals: Work with the Patient Care Coordinator to make referrals to appropriate disciplines as needed.
  • Provide Education and Supervision: Offer in-service education, healthcare professional education, and public education, while supervising peers and supporting personnel and students in training as needed.
  • Conduct Research and Orientation: Conduct research and assist with the orientation of new employees as required.

Requirements:

  • Education: Minimum bachelor's degree in physical therapy from an accredited school of physical therapy and current licensure from the state where therapy will be provided.
  • Experience: Prefer a minimum of one year's therapeutic experience in a non-home healthcare environment.
  • Knowledge and Skills: Strong knowledge of theory and concepts relating to the practice of physical therapy, with the ability to evaluate and treat patients for whom a physician's order exists.
  • Continuing Education: Minimum of twelve hours of continuing education annually.
  • Travel and Working Conditions: Accept travel and working conditions as assigned.
