Role Summary:

The Social Services Manager is responsible for providing essential support to residents and their families, advocating for patient needs, and facilitating discharge and advanced planning processes. This role requires strong organizational skills, quick thinking, and a friendly approach.

Key Responsibilities:

  • Conduct assessments of residents upon admission, during changes in condition, and at regular intervals.
  • Develop and maintain individualized care plans, reviewing and updating as necessary.
  • Ensure thorough clinical documentation of resident interactions and progress notes.
  • Coordinate resident appointments with external services, including dental and optical care, while collaborating with residents, families, and team members for discharge planning.
  • Possess knowledge of psycho-social practices relevant to long-term care settings.
  • Communicate effectively with residents, families, and all organizational levels while maintaining confidentiality.
  • Embrace the responsibility of caring for residents, families, visitors, and all individuals encountered in the course of work.

Qualifications:

  • Master's Degree preferred.
  • Licensure as a Social Worker (LSW/LCSW) is not mandatory.
  • A minimum of three years of experience in long-term care is preferred, with management and supervisory experience recommended.
  • Strong written and verbal communication skills.
  • Interpersonal skills necessary for effective communication with guests and representing the facility.
  • A kind demeanor and professional appearance are essential.
  • Ability to work independently when required.
  • Dependable and punctual.
  • Flexible availability to meet the needs of residents and the facility.
  • Completion of a physical/essential functions test or fit-for-duty physical may be required.
  • Provision of references from previous employers.
  • Submission to a criminal background check as per regulatory requirements.
