Position Overview:
PACE of Southwest Michigan is actively seeking a full-time Case Manager (MSW) to enhance our Social Work team.

We are in search of a proactive individual who thrives on fostering connections with our participants and their caregivers within a collaborative care framework.

The Case Manager (MSW) will be tasked with orchestrating and supervising the provision of healthcare services to elderly participants. This includes conducting thorough assessments, formulating care plans, collaborating with the interdisciplinary care team, and advocating for the needs of our participants.

This role operates under the supervision of the Social Work Supervisor while possessing the autonomy to apply advanced social work expertise and skills.

About PACE of Southwest Michigan:
PACE (Program for All-Inclusive Care for the Elderly) of Southwest Michigan is a 501(c)(3) organization committed to uniquely bridging the health and social supports necessary to uphold the independence and dignity of our participants.

We deliver essential medical treatment, support, and social engagement to empower our participants to remain in their homes for as long as feasible.

The care team comprises physicians, nurses, pharmacists, social workers, therapists, and transportation staff who collaborate daily to ensure comprehensive care.

Since its inception, PACE has expanded its team to over 90 professionals and currently serves more than 250 participants and their families.

The distinctive PACE model of care is founded on the principle that seniors with chronic care needs benefit most from being served in their homes and communities whenever possible.

Key Responsibilities:
  • Engage as a member of the Interdisciplinary Team (IDT) to conduct initial assessments, develop care plans, and perform periodic reassessments.
  • Communicate participant updates to the IDT and relevant parties.
  • Document updates in the participant's medical record, including assessments and behavior modification plans.
  • Coordinate admissions and discharges from facilities as deemed appropriate by the IDT.
  • Refer participants and families to suitable community agencies or facilities and act as a liaison and advocate.
  • Participate in program and policy development and contribute to the Quality Improvement Plan process.
  • Facilitate the Medicaid application and redetermination process with the Michigan Department of Health and Human Services (MDHHS).
  • Complete home visits as necessary.
  • Adhere to all established policies, rules, and regulations.
  • Perform other duties as assigned within the job scope.
Qualifications:
  • Master's degree in Social Work.
  • Two years of experience in a health-related field is preferred.
  • Licensed MSW in the State of Michigan, valid Driver's License with a safe driving record, and CPR certification upon hire.
Skills and Knowledge:
  • High level of autonomy required for clinical decisions.
  • Excellent attention to detail and organizational abilities.
  • Strong verbal and written communication skills.
  • Ability to work a flexible schedule.
  • Knowledge of the physical, mental, and social needs of frail older adults.
  • Capable of working both independently and within a team.
  • Self-motivated with strong decision-making skills.
  • Proficient with technology and quick to learn new software.
