Job Summary

We are seeking a skilled Patient Care Specialist to join our team at SSM Health. As a Patient Care Specialist, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities
  • Assist in performing a variety of nursing care services for patients, including bathing, ambulating, feeding, and personal and oral hygiene.
  • Collect and document data for the admission assessment and reassessment at specific times for patient population.
  • Contribute to safety and quality care by relaying information via charting and communicating to team members.
  • Inform licensed nurse of any changes in patient condition/needs, of patient complaints, and of patient/significant other educational needs.
  • Perform mechanical and manual operation of equipment as recommended in user's manual, with ability to recognize common mechanical problems and initiate appropriate corrective action.
  • Work in a constant state of alertness and safe manner.
Requirements
  • Passing grade on state of work location required exam.
  • Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
  • Certified Nursing Assistant (CNA) - Wisconsin Department of Health Services or Certified Nursing Assistant (CNA) - Wisconsin Nurse Aide Registry
Work Environment

This is a full-time night shift position working 12 hour shifts from 7:00 p.m. to 7:30 a.m. for a total of 36 hours per week. The schedule also includes an every third weekend rotation and a holiday rotation.
