Job Summary:

Liberty Health is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Nurse Specialist, you will play a vital role in providing compassionate care to patients and their families facing terminal illness.

Responsibilities:

  • Apply a working knowledge of nursing theories and concepts to provide high-quality patient care.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure seamless care delivery.
  • Communicate effectively with patients/families at all levels, ensuring their needs are met and concerns addressed.
  • Collaborate with all agency personnel to ensure comprehensive care and support.
  • Participate in Interdisciplinary Team (IDT) meetings and quality reviews to drive continuous improvement.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience, with a minimum of one year in hospice nursing.
  • Knowledge of the framework, function, and organization of a home health and hospice care program.
  • Working knowledge of nursing principles and practices, as well as reimbursement guidelines.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and work flexible hours, including weekends and holidays.
