Work Schedule:
Post-orientation, the typical schedule is Monday to Friday, 8 AM to 5 PM, with flexibility required based on patient needs and emergencies. A dedicated on-call team is available, and you will rotate as backup on-call with your colleagues.

Patient Caseload and Visits:
As a registered nurse, you will manage an average caseload of 13-15 patients, with potential fluctuations based on referral patterns. Our aim is to achieve a minimum of 20 visits per week, averaging 4-5 visits daily, while accommodating patient needs and locations.

Visits generally last between 45 minutes to 1 hour, with longer durations for active symptom management to ensure patient comfort.


Key Responsibilities:
You will provide care to patients in their chosen environment, whether at home, in assisted living, or skilled nursing facilities. Your role will involve training caregivers on essential care practices, including wound care, medication administration, and personal care techniques, as well as preparing them for end-of-life expectations.

Expect to spend considerable time traveling between patient visits, documenting care, and coordinating with healthcare providers. You will utilize your personal vehicle, with mileage reimbursement available for travel between visits.

Care coordination involves communication with community medical providers, hospice teams, and other disciplines to ensure comprehensive care. Patient assignments are made based on team needs, acuity, and geographic considerations.

Documentation will be completed in real-time during patient visits, with online ordering for medications and supplies.

Self-scheduling is based on patient needs, requiring flexibility to assist your team in delivering exceptional care.

You will collaborate within an Interdisciplinary Team (IDT) that includes a physician, nurse practitioner, registered nurse, medical social worker, chaplain, and a CNA, with volunteer support available for patient needs. The team convenes bi-weekly to discuss patient eligibility.


Qualifications:
  1. Education: Graduate of an accredited nursing program (AD, Diploma, or BSN).
  2. Licensure: Current nursing license in the State of North Carolina.
  3. Experience: Minimum of two years of nursing experience, preferably in home care, hospice, or related oncology settings.
