Job Summary:

Liberty Health is seeking an experienced Hospice Nurse to join our team. As a Hospice Nurse, you will provide state-of-the-art care and pain management services to patients with terminal illnesses.

Key Responsibilities:

  • Apply a working knowledge of nursing theories and concepts to provide high-quality patient care.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure best practices and patient-centered care.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience and one year of hospice nursing experience preferred.
  • Knowledge of home health and hospice care programs and the nurse's role as a member of the patient care team.
  • Working knowledge of nursing principles and practices according to nursing practice act, agency policies, and procedures.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and work flexible hours, including weekends, evenings, and holidays.
