Position Overview

Responsibilities of the Hospice RN / RN Case Manager

  • Report directly to the Administrator, Executive Director, or Patient Care Manager, maintaining primary oversight and professional management of each patient.
  • Serve as the main point of contact between the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients while offering supportive care to their families, managing a reasonable caseload and fostering a trusting, compassionate relationship.
  • Empower caregivers by providing education on how to care for their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to ensure comfort, peace, and dignity for patients.
  • Assess patient and family needs, assisting in the development and implementation of personalized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Engage in company quality assessment and performance improvement initiatives.

Candidate Profile

Qualifications for the Hospice Registered Nurse / RN Case Manager:

  • A compassionate heart: empathetic and kind towards patients and their families.
  • Comfortable providing care in various home settings.
  • Exhibit flexibility and composure while supporting others during challenging times.
  • Possess an RN license in the state of employment (or eligibility to obtain).
  • Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is strongly preferred.
  • Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
  • Valid driver's license and automobile insurance are required.
  • Current CPR certification is necessary.
