About the Role

Amedisys is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing patient-centered care and collaborating with our interdisciplinary team to develop and implement individualized plans of care.

Responsibilities
  • Perform comprehensive patient assessments and develop plans of care that address the unique needs and goals of each patient.
  • Collaborate with the care team to ensure seamless coordination of care and effective communication with patients, families, and healthcare providers.
  • Provide education and support to patients and their families to promote health, independence, and self-care.
  • Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure high-quality care and adherence to Amedisys standards.
  • Document patient progress and outcomes in a timely and accurate manner, ensuring compliance with regulatory requirements and Amedisys policies.
  • Participate in ongoing education and professional development to stay current with best practices and industry developments.
Requirements
  • One (1+) year of clinical experience as a Registered Nurse (RN) in a home health or similar setting.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
