Key Responsibilities as a Hospice RN / RN Case Manager

  • Directly report to the Administrator, Executive Director, or Patient Care Manager, maintaining oversight and professional management of each patient.
  • Serve as the primary point of contact between the physician, patient/family, and hospice team.
  • Deliver direct care services to hospice patients while providing supportive care to their families, ensuring a compassionate and trusting relationship.
  • Empower caregivers by educating them on how to care for their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort and dignity.
  • Assess patient and family needs, assisting in the development and implementation of individualized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives within the organization.

Candidate Profile

Requirements for Hospice Registered Nurse / RN Case Manager:

  • A compassionate and empathetic approach to patient care.
  • Comfortable providing care in various settings as per patient needs.
  • Ability to remain calm and flexible while supporting others during challenging times.
  • Valid RN license in the state of employment (or eligibility to obtain).
  • Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings such as Med Surg, PACU, ER, critical care, or travel nursing is advantageous.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
