Key Responsibilities of the Hospice RN/RN Case Manager
- Report directly to the Administrator, Executive Director, or Patient Care Manager, maintaining primary oversight and professional management of each patient.
- Serve as the main point of contact between the physician, patient/family, and hospice team.
- Deliver direct patient care services to hospice patients and provide supportive care to their families, fostering a trusting and compassionate relationship.
- Empower caregivers by educating them on how to care for their loved ones.
- Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
- Assess patient and family needs and assist in developing and implementing the care plan for each patient.
- Maintain all assigned patient clinical records accurately.
- Engage in company quality assessment and performance improvement initiatives.
Qualifications for Hospice Registered Nurse/RN Case Manager:
- A compassionate heart: kind, empathetic, and understanding towards patients and their families.
- Comfortable providing care in various settings.
- Exhibit flexibility and composure while supporting others during challenging times.
- Possess an RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatrics, or oncology is strongly preferred.
- Additional preferred experience includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
- Hold a current driver's license and automobile insurance.
- Maintain current CPR certification.
