Nurse Care Specialist

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Consulate Health Care. As an LPN, you will play a crucial role in providing high-quality nursing care to our residents. You will have the opportunity to collaborate with our healthcare team to implement individualized treatment plans for each assigned resident, ensuring their unique needs are satisfied.
Responsibilities
As an LPN, your responsibilities will include:
• Supervising and evaluating Certified Nursing Assistants (CNAs) to ensure they are delivering care following our high standards.
• Overseeing our fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions.
• Conducting thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.
Requirements
To be successful in this role, you will need:
• An active license to practice as an LPN.
• Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
