Job SummaryWe are seeking a highly skilled Clinical Care Specialist to join our team. This is a challenging and rewarding role that requires strong clinical skills, excellent communication, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Patient Assessment and Care: Conduct comprehensive assessments of patients to identify their individual needs and develop personalized care plans.
- Medication Administration: Administer medications accurately and safely, monitoring patients for any adverse reactions or side effects.
- Care Coordination: Work closely with other healthcare professionals to ensure seamless care transitions and coordination.
- Educational Support: Provide educational support to patients and families on health-related topics, promoting self-management and empowerment.
- Education: Graduate of an accredited school of nursing, verified by state licensure.
- Experience: At least 2 years of experience in a hospital or medically related environment, desirable.
- Licenses/Certificates: Current Tennessee or Compact State registered nurse license required, current BLS and SAMA certifications also necessary.
We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package including health insurance, retirement savings plan, and paid time off.
