**Job Title:** Clinical Care Manager - Patient Advocacy

**Estimated Salary Range:** $85,000 - $115,000 per year

We are seeking a dedicated and compassionate Clinical Care Manager to join our team at Hamilton Health Care System. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care.

About the Role:

  • Develop and implement individualized care plans for patients, taking into account their unique needs and preferences.
  • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless communication and coordination of care.
  • Conduct thorough assessments of patients' medical and social histories, identifying potential risks and developing strategies to mitigate them.
  • Maintain accurate and up-to-date records of patient interactions, treatment plans, and outcomes.

Requirements:

  • A current Tennessee or Compact State registered nurse license is required.
  • At least two years of experience in a hospital or medical environment is preferred.
  • BLS and ACLS certifications are mandatory; if not certified upon hire, must acquire within 7 days and one year, respectively.
