Job Title: LPN (Licensed Practical Nurse)

At Holmes Lake Rehabilitation & Care Center, we are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare staff, you will play a vital role in delivering high-quality care to our residents.

Job Summary:

As an LPN at Holmes Lake, you will be responsible for assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents.

Responsibilities:
  • Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
  • Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
  • Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
  • Communicate effectively with residents, families, and the healthcare team to ensure seamless care coordination.
  • Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
  • Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
Qualifications:
  • Valid LPN license in NE.
  • Graduation from an accredited LPN program.
  • Previous experience in a skilled nursing facility or similar healthcare setting preferred.
  • Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
  • Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
