Job Summary

We are seeking a highly skilled and compassionate Behavioral Health RN Case Manager to join our team. As a key member of our healthcare team, you will be responsible for managing a caseload of behavioral health patients, assessing their health, psychosocial needs, and ensuring they receive the care they deserve.

Key Responsibilities:
  • Manage patient care plans, adapting as needed to meet changing patient conditions.
  • Collaborate with an interdisciplinary team to craft personalized care plans.
  • Partner with physicians, clinical supervisors, and healthcare team members to ensure top-quality care and timely interventions.
  • Empower patients, families, and facility staff through education and support, enhancing the care experience.
  • Supervise and mentor behavioral health aides and LPNs, ensuring consistent, high-quality care delivery.
