Exciting Opportunity for a Home Health Physical Therapist

BAYADA Home Health Care is seeking an experienced and skilled Physical Therapist to join our team in Charlton, Massachusetts. This full-time position offers a competitive salary of $115,000 per year and a great benefits package.

About the Job:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to adult and geriatric patients in their homes. As a home care physical therapist, you will be part of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients, allowing them to maintain their independence.

Responsibilities:

  • Making visits to clients in their homes to assess their needs and develop personalized care plans.
  • Performing diagnostic tests and measurements to evaluate mobility, strength, endurance, balance, and other aspects of patient health.
  • Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance interventions, and other therapies as needed.
  • Continuously assessing and revising the Physical Therapy care plan to ensure optimal outcomes.
  • Educating and instructing clients, family members, or caregivers on rehabilitative care and activities necessary for promoting client health, safety, and independent living.
  • Accurately documenting observations, interventions, and evaluations related to client care management and services provided.

Requirements:

  • A current license as a Physical Therapist in Massachusetts.
  • A minimum of one year of recent clinical experience as a licensed Physical Therapist.
