We are seeking a highly skilled Nurse Care Coordinator to join our team at the Department of Veterans Affairs. As a Nurse Care Coordinator, you will play a critical role in providing comprehensive care to our nation's veterans.

The salary for this position is estimated to be between $61,869 and $121,930 per year, based on location and experience.

Job Summary:

This is a full-time, permanent position with the Department of Veterans Affairs, working in the Lexington VA Health Care System. The successful candidate will work as part of an interdisciplinary team to provide care to veterans in a telehealth setting.

The ideal candidate will have excellent communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.

Duties and Responsibilities:

  • Provide initial and ongoing comprehensive assessments to include review of systems and establish a comprehensive plan of care.
  • Use the treatment plan to assess, identify, analyze, and prioritize problems, interventions, and SMART goals.
  • Triage and assess all data received from Home Telehealth patients such as vital signs, reported symptoms, and question responses.
  • Reviews all patient responses each work day and contacts patients with high risk responses and trends, significant changes in condition or changes in other specific data elements received as clinically appropriate.
  • Identify and intervenes for potential exacerbations or complications to facilitate timely care in clinic, emergency response/urgent care, or care in the community.
  • Provide appropriate intervention such as medication management, case management, and patient education.
  • Triage incoming calls and concerns of patients/families, resolve those within scope of practice and route to other interdisciplinary team staff or services.
  • Provide communication and data exchange with community-based providers when there is co-managed care.
  • Identifies patient knowledge, health factors, skills and behaviors that support self-management.
  • Provides health care coaching, patient education, and psychosocial support.
  • Document and communicate with PACT and Specialty Clinics providers/teams regarding changes in status: progress toward goals; patterns or trends in data: symptoms or findings of concern and need for provider assessment and/or interventions.
  • Facilitate, document and communicate treatment changes to the Veterans as directed by providers. Provide follow-up evaluation of the Veteran after changes are implemented.
  • Maintains patient panel size consistent with local and national policy/directives.
  • Provides leadership in performance improvement activities, implementation of new telehealth program initiatives, and Nursing Service level activities.
  • 16. Other duties, as assigned by the Nurse Manager, CC/HT.

Requirements and Qualifications:

To be eligible for this position, you must meet the following qualifications:

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • English Language Proficiency.
  • At least one year of specialized nursing experience equivalent to Nurse I, Level 1.
  • A Bachelor of Science in Nursing (BSN) degree.
  • Other relevant qualifications may be considered.
