Job Summary
">We are seeking a skilled Nurse to join our team at Bluegrass Care Navigators. As a Hospice Nurse, you will provide compassionate care and support to patients and their families, helping them navigate the complexities of end-of-life care.

">The successful candidate will have a strong foundation in nursing practice, with a focus on pain management, wound care, and symptom control. You will work closely with patients, families, and other healthcare professionals to develop personalized care plans that meet the unique needs of each individual.

- Provide skilled nursing care to patients in their homes, hospices, or hospitals">
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs">
- Develop and implement individualized care plans that address patients' complex health needs">
- Work collaboratively with interdisciplinary teams to ensure seamless care coordination">
- Maintain accurate records of patient care and communicate effectively with patients, families, and healthcare providers">
- Prioritize tasks efficiently, managing time and resources to meet patient needs">
Requirements
">To be considered for this role, applicants must:
">- ">
- Hold a current RN license and a valid driver's license">
- Have a minimum of 1 year of experience in hospice, home health, medical-surgical, or critical care nursing">
- Demonstrate excellent communication, observation, and documentation skills">
- Show empathy and compassion when working with patients and families experiencing grief and loss">
- Be able to lift up to 25 pounds, stand for extended periods, and travel to patient locations as needed">
