Job Summary

Wexford Health Sources, Inc. is seeking a Licensed Practical Nurse (LPN) to join our team at the Logan Correctional Center in Lincoln, IL. As an LPN, you will play a vital role in providing high-quality patient care to our justice-involved patients.

Responsibilities
  • Assist in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
  • Under the supervision of a Physician or Registered Nurse, perform non-professional nursing care duties in the assessment of the physical, psychological, and social dimensions of patients.
  • Assist in planning individual treatment programs by utilizing available resources in planning care and consult with registered nurses, physicians, and correctional officers.
  • Implement individual treatment programs in the Health Care Unit and, as necessary, in the housing unit.
  • Assess personal and learning development needs and plan and implement programs to meet identified needs.
Requirements
  • Currently licensed as an LPN in the state of Illinois.
  • Current CPR certification from the American Red Cross, American Heart Association, or American Safety and Health Institute.
  • Graduate from an accredited School of Nursing.
  • One year clinical experience, prior experience with use of electronic medical record system preferred.
