Our Wound Care Coordinator plays a crucial role in enhancing the quality of life for our residents. This specialized nursing position focuses on the assessment and treatment of patients with intricate wounds, ostomies, and continence issues. The Wound Care Coordinator collaborates with an interdisciplinary team to ensure that individualized treatment strategies are effectively implemented, aiming to avert future complications.

Key Responsibilities:

  • Conduct thorough assessments and develop tailored care plans for residents with complex wound needs.
  • Engage in discharge planning and contribute to the formulation and execution of resident care strategies.
  • Maintain accurate documentation and records for MDS assessments and restorative nursing interventions.
  • Foster a compassionate environment, focusing on the needs of seniors, including those with Alzheimer's and Dementia.
  • Promote teamwork and a positive workplace culture, encouraging innovation in care practices.
  • Oversee the organization and implementation of restorative care services in compliance with relevant regulations.
