Physical Therapist

The Physical Therapist reflects the mission, vision, and values of Northwestern Memorial Healthcare, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
Responsibilities:- Develops and Administers Physical Therapy Evaluations and Treatment Plans: Provides assessments and selects appropriate techniques and therapies to restore function and prevent disability.
- Selects and Administers Appropriate Physical Therapy Evaluations and Treatment Plans: Provides patient, family, and caregiver education.
- Participates in Patient Discharge Planning: Verbal and written communications with other team members.
- Provides Clear and Concise Patient Care Documentation: Departmental standards.
- Supervises Paraprofessionals and Support Personnel: Monitors treatment plans' administration and provides follow-up clinical direction and guidance.
- Supervises Students: Departmental clinical education guidelines.
- Administration of Work:
- Provides Direct Patient Care: 65% of hours worked.
- Performs Physical Therapy Evaluations and Re-Evaluations: Regular basis.
- Designs and Implements Treatment Plans:
- Participates in Patient Charge and Home Care Planning: Facilitates continuity of patient care.
- Completes Medical Documentation: Timely and accurate manner, established policies, and standards.
- Assists in Monitoring Quality Management Indicators:
- Demonstrates Competency: Annual completion of written and demonstration style competency examinations.
- Leadership/Supervisory Skills:
- Delegates Responsibilities: Subordinates.
- Promotes Philosophy and Objectives: Northwestern Memorial Healthcare and Rehabilitation Services.
- Presents In-Service: Rehabilitation Services Department and/or Northwestern Memorial Healthcare staff.
- Completes Special Projects: Assigned by the Manager or Clinical Specialist.
- Communications/Relationships:
- Promotes Professional Working Relationships: Peers.
- Maintains/Open Communication: Internal contacts, nursing, and social work.
- Participates in Mentorship Program:
- Participates in Interdisciplinary Rounds:
- Serves on Departmental Committees:

- Physical/Visual/Mental Qualifications:
- Provides Clear and Effective Communication: Physicians, patients, families, guests, and healthcare team.
- Responds to Emergency Signals:
- Pushes/Pulls Patients and Supplies: Short distances.
- Lifts Patients and Supplies: Up to 80 lbs. or with assistance up to 200 lbs.
- Visually Assesses and Monitors Patients:
- Observes and Comprehends Written Format: Medical chart.
- Provides Physical Assistance: Patients during mobility training.
- Determines Patients' Verbal Responses: Evaluation and treatment activities.
- Displays Concentration and Attention Span: Brief and extended periods of time.
- Commutes Between Office Space and Patient Treatment Locations:
This position is full-time, 80 hours per pay period, Monday - Friday. No holidays or weekends at this time, but that may change in the future. Hours vary due to clinic hours. Flexible hours required between 7:00 a.m. to 6:00 p.m. to meet department needs. Typical start time 7:00 a.m. to 8:00 a.m.
Experience in wound care is mandatory.
Certifications in lymphedema and wound care preferred.
