OVERVIEW:
We are seeking a dedicated Licensed Practical Nurse Coordinator to join our compassionate team. In this role, you will be instrumental in delivering high-quality care to patients in their residences, ensuring their comfort and well-being.
KEY RESPONSIBILITIES:
Under the guidance of the Senior RN Care Coordinator and the Scheduling Coordinator, your primary duties will include:
- Providing skilled nursing care to patients in home settings.
- Collaborating with healthcare professionals to develop and implement patient care plans.
- Monitoring patient conditions and reporting any changes to the supervising RN.
- Administering medications and treatments as prescribed.
QUALIFICATIONS:
To be considered for this position, candidates must meet the following criteria:
- Possess a current nursing license and registration in the state of New York.
- Have at least one year of experience in a skilled nursing facility, home care, or hospital setting.
- Six months of hospice experience is preferred.
- Ability to lift a minimum of 20 pounds and perform necessary physical tasks.
- Demonstrate effective verbal and written communication skills.
- Hold a valid driver's license and have access to a reliable vehicle with current insurance.
- Successfully complete the NLN proficiency exam in medication administration.
WORK SCHEDULE:
This is a full-time position with expected hours of 30 per week. The standard shift will be during the day, Monday through Friday.
COMPENSATION:
The pay range for this role is $19.50 – $24.00 per hour, commensurate with experience.
