Job Description
This role is ideal for a registered nurse who is passionate about patient care coordination and advocacy. As a Care Manager, you will work closely with the hospital care team and patients' families to ensure smooth transitions and high-quality clinical and cost outcomes. You will be responsible for procuring and securing post-acute services, coordinating and advocating for patients and families with internal and external stakeholders, and identifying potential barriers to care coordination/discharge planning.
Responsibilities and Expectations
- Coordinate patient care from admission through discharge, ensuring smooth transitions and high-quality clinical and cost outcomes
- Work with the hospital care team and patients' families to plan and implement the best possible plan for the patient, taking into account various factors, limitations, and patient/family preferences
- Identify and recommend post-acute services and complete referrals to appropriate providers in a timely manner, coordinating directly with patients' families and the care team
- Ensure alignment and appropriateness of post-acute services as patients clinically progress throughout their stay
- Participate in the development of strategies to reduce unnecessary length of stay and/or resource consumption
