RN Case Manager Hospice Specialist

We are seeking a compassionate and skilled RN Case Manager to join our team at Assured Hospice of Centralia, a part of LHC Group. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
About the RoleAs a Hospice RN Case Manager, you will be responsible for:
- Conducting initial nursing evaluations to determine eligibility for hospice services
- Identifying patients' physical, psychosocial, and environmental needs and reassessing them regularly
- Developing and implementing individualized plans of care in collaboration with patients, families, and healthcare teams
- Documenting patient progress and outcomes
To be successful in this role, you will need:
- One year of clinical experience as a registered nurse
- Current RN licensure in the state of practice
- Current CPR certification
- A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
