RN Case Manager

We are seeking a skilled RN Case Manager to join our Hospice team at Assured Home Health & Hospice. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality patient care and support to individuals and their families during their end-of-life journey.
Key Responsibilities- Initial Nursing Evaluation: Conduct thorough nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Patient Assessment: Identify patients' physical, psychosocial, and environmental needs, and reassess as needed, no less than every 15 days.
- Plan of Care Development: Initiate and coordinate the plan of care, maintaining accurate and relevant clinical notes regarding the patient's condition.
- Collaboration and Communication: Collaborate with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) to provide patient and family care daily.
- Experience: One year of clinical experience.
- Licensure: Current RN licensure in the state of practice and one year of clinical experience.
- Certifications: Current CPR Certification.
- Transportation: Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
