About the RoleAs a Hospice RN Case Manager, you will be responsible for delivering high-quality end-of-life care to our patients. This will involve conducting initial nursing evaluations, identifying patient needs, and developing and implementing individualized plans of care.
Key Responsibilities- Conduct initial nursing evaluations to determine patient eligibility for hospice services
- Identify patient needs and re-assess as needed, with a focus on delivering high-quality care
- Develop and implement individualized plans of care, with a focus on patient-centered care
- Collaborate with patients, families, and healthcare teams to ensure seamless care delivery
- Current RN licensure in the state of practice
- One year of clinical experience
- Current CPR Certification
- Valid driver's license and access to a dependable vehicle
