We are seeking a skilled RN Care Manager to join our team at Assured Home Health & Hospice. As a RN Care Manager, you will play a vital role in providing quality care to our patients in their homes.
About the RoleThe RN Care Manager is responsible for the supervision and coordination of clinical services, ensuring that patients receive the highest level of care. You will work closely with our healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities- Provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
