POSITION SUMMARY: The role involves delivering integrated behavioral health services that complement primary care and mental health initiatives. This position focuses on prevention, intervention, and recovery strategies for mental health and substance use disorders within a primary care framework.

KEY RESPONSIBILITIES:

Analytical and Critical Thinking:

  • Administer brief, evidence-based screenings tailored to developmental stages.
  • Conduct focused assessments within a limited timeframe.
  • Utilize evidence-based practices, including assessment and intervention methodologies, to support individuals, couples, families, and groups across various age demographics.
  • Provide immediate evaluations for patients in crisis and coordinate psychiatric hospitalizations when necessary.
  • Evaluate discharge readiness throughout the treatment process.

Problem Solving and Decision Making:

  • Develop and execute integrated treatment plans, ensuring timely access to both internal and external resources.
  • Independently assess treatment progress and adjust plans as necessary, even without predefined outcome measures.

Communication:

  • Offer consultation to medical staff and other service providers to ensure a seamless continuum of care.
  • Engage in required staff meetings.
  • Maintain clear and effective verbal, nonverbal, and written communication.
  • Contribute to system improvements and advocate for a healthy workplace.
  • Ensure that progress notes are accessible to the patient's care team and consider patient accessibility.

Patient Engagement:

  • Facilitate referrals to both internal and external services.
  • Demonstrate skills in engaging patients and families who may be reluctant to participate in services through proactive outreach and follow-up.

Compliance and Documentation:

  • Document all behavioral health services in accordance with state regulations and professional standards.
  • Adhere to departmental productivity benchmarks and established policies.
  • If supervising, demonstrate understanding of supervision models and effectively address competency limits.
  • Utilize standardized measures for risk assessment.
  • Complete all required policy training.
  • Ensure proper billing practices for all clinical documentation.

Technology Use:

  • Leverage Electronic Health Records to enhance integrated healthcare delivery.

Licensure Requirements:

  • Fulfill mandatory continuing education requirements to maintain licensure/certification.
  • Perform additional duties as assigned by the supervisor.

SKILLS AND QUALIFICATIONS:

  • Ability to provide culturally relevant services and address healthcare disparities.
  • Adapt services to align with cultural norms and promote diversity within interprofessional teams.
  • Familiarity with open access care models in community mental health settings.
  • Adaptability to changes in the healthcare landscape at both state and national levels.

EDUCATION AND EXPERIENCE:

  • Required: Master's Degree in Social Work, Counseling, Psy D, or PhD.
  • Preferred: Licensure such as LMSW, LCSW, LCSW-C, LGPC, LCPC, LMFT, LCMFT.
  • Licensure Required: Must be licensed in the state of Maryland.
  • Preferred: Minimum of two years of experience providing outpatient mental health services to diverse populations, with a strong preference for experience in primary care settings.
