Key Responsibilities
  • Initial Appointments: Conduct initial appointments with potential participants and their caregivers to describe the PACE model and its benefits.
  • Eligibility Evaluation: Ensure potential participants meet the eligibility criteria for the PACE program in accordance with federal regulations.
  • Needs Assessment: Use a strength-based approach to evaluate the current needs and health status of potential participants.
  • Communication: Proactively communicate with the interdisciplinary team regarding the enrollment status and progress of prospective participants.
  • Mediation: Mediate and prioritize between the needs of prospective participants and caregivers and the concerns of the interdisciplinary team.
  • Data Management: Maintain accurate data on new referrals in the CRM, EMR software, and Masshealth Virtual Gateway.
  • Marketing Support: Coordinate and assist with marketing and outreach efforts, including outside events.
Requirements
  • Education: Associate's or Bachelor's degree in Nursing, or completion of a Nursing Diploma Program.
  • Licensure: Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
  • Experience: Minimum one (1) year experience in a geriatric healthcare and/or adult mental health setting.
  • Skills: Advanced computer skills, including Microsoft Office, Customer Relationship Management, and Electronic Medical Records software.
  • Qualifications: Ability to communicate effectively with and establish working relationships with various medical licensed personnel.
  • Certifications: Covid vaccine required.
