Job Summary

We are seeking a skilled Triage Nurse to join our team at Northwestern Memorial Healthcare. As a Triage Nurse, you will play a critical role in providing exceptional patient care and ensuring seamless communication between patients and healthcare providers.

Key Responsibilities
  • Provide clinically expert triage service, assessing and determining the acuity of patients' symptoms and communicating these to healthcare providers as appropriate.
  • Establish positive first impressions with patients and customers, serving as a patient advocate and liaison between patients and healthcare providers.
  • Triage all incoming telephone calls and electronic messages, prioritizing and resolving, involving healthcare providers as necessary.
  • Document all communications in the electronic medical record and schedule patient appointments as needed.
  • Assist in coordinating emergency and hospital transfers, including paperwork and telephone communications.
  • Administer medications and treatments in accordance with Nursing Standards and as ordered by healthcare providers.
  • Provide education to patients and staff concerning clinical issues and assist in coordinating and monitoring activities of clinical staff to ensure adequate coverage and adherence to OSHA and CDC guidelines.
  • Perform other duties as assigned, including assisting in specific process improvement projects and participating in departmental quality improvement activities.
Requirements
  • Bilingual: English/Spanish preferred.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent customer service and communication skills.
  • Ability to function as part of a highly effective team that consistently puts patients first.
Working Conditions

This role will involve care and management of the abdominal post-transplant population and will not involve hands-on patient care. The role will work on facilitating transitioning of patients to their local providers.
