Job Summary

We are seeking a highly skilled and compassionate Post Liver Transplant Coordinator to join our team at The University of Chicago Medicine. As a key member of our Liver Transplant Program, you will play a vital role in providing exceptional patient care and coordination services.

Key Responsibilities
  • Provide patient education and support to pre-transplant recipients, ensuring they understand their treatment plans and options.
  • Coordinate meetings with patients and healthcare teams to review treatment plans and progress.
  • Monitor patient progress, answer questions, and provide guidance on pre-operative care.
  • Participate in discharge planning with the multidisciplinary team to ensure seamless transitions.
  • Evaluate patient compliance with immunosuppressive regimens and provide education on medication management.
  • Coordinate scheduling of pre-transplant medical procedures, diagnostic testing, and clinic visits.
  • Communicate with physicians and healthcare teams to ensure timely and effective care.
  • Notify transplant team members of pre-transplant admissions and coordinate patient admissions as needed.
  • Provide ongoing pre- and post-transplant care maintenance, including medication management and authorization.
  • Collect and document patient data, including UNOS forms, as required.
  • Participate in patient-related consultations with physician staff and attend multidisciplinary meetings/rounds.
Requirements
  • Associate Degree in Nursing; BSN degree or higher preferred.
  • Current licensure as a Registered Nurse in the State of Illinois.
  • Excellent decision-making and communication skills.
  • Effective teaching skills and ability to work with undergraduate and post-graduate nursing students.
  • Basic computer skills and familiarity with insurance coverage and managed care contracts.
Preferred Qualifications
  • Master's in Nursing.
  • Five years' experience working with transplant patients.
  • A combination of transplant and other related nursing experience.
