Job Summary

We are seeking a skilled Case Manager II to join our team at Encompass Health. As a Case Manager II, you will play a vital role in coordinating patient care from admission to post-discharge, ensuring seamless transitions and optimal outcomes.

Key Responsibilities
  • Coordinate patient care with interdisciplinary teams to develop and implement individualized treatment plans
  • Monitor patient progress and adjust plans as needed to ensure timely and effective care
  • Communicate with patients, families, and healthcare providers to ensure informed decision-making and optimal care
  • Manage resources and coordinate logistics to ensure efficient and effective care delivery
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • 2+ years of experience in case management or a related field
  • Strong clinical knowledge and skills, with ability to analyze complex patient data and develop effective care plans
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
