Job Summary

Under the supervision of the Program Manager and/or Team Leader, this Case Manager will provide case management, rehabilitation, and crisis interventions in home office and community settings to adults with serious long-term mental illness.

The ideal candidate will collaborate with psychiatric and allied mental health teams in assessment, diagnosis, intervention, and treatment planning.

This role will also involve conducting substance abuse treatment with clients in need, in accordance with the 12 core functions of substance abuse counseling as defined by the ICRC.

Key Responsibilities
  • Provides clinical interventions, including resource linking, resource modification, and development skill teaching and programming to consumers on the Mobile Treatment Team caseload.
  • Assists consumers in accessing medical and dental care, employment, housing, and social support networks.
  • Conducts comprehensive clinical assessments and develops interventions for substance abuse, sexual abuse, symptom management, and family support and education.
  • Assesses suicidal and homicidal behavior and coordinates emergency psychiatric services.
  • Provides and coordinates treatment services for assigned cases, including documentation in progress notes and treatment plans.
Requirements
  • Bachelor's degree in a Human Services field, such as Social Work, Psychology, or Rehabilitation Services.
  • Two years of experience in Human Services, preferably working with individuals with substance abuse, sexual abuse, and/or involvement with the criminal justice system.
  • Valid driver's license and use of a car during normal working hours or other means of transportation to complete major functions of the job.
